PROGRAM CONTRACT

Submit your program to MVTV. Complete the program contract form to schedule your show.

By completing this form, you assume full responsibility for the program contents pursuant to all applicable laws and statutes. You must complete and sign this form for us to schedule your program. MVTV may use portions of this program for non-commercial and/or promotional purposes. All programs must have opening and closing credits that list the title of the program and the name of the program’s producer or submitter.

*I understand and agree that electronically typing my name constitutes a legally binding signature. I agree that the electronically stored copy of my signature, authorizations, and documents will be considered the true, accurate, and enforceable record. I will not contest the admissibility or enforceability of these documents.

I understand that all of the equipment listed above is to be used for the sole purpose of producing non-commercial programs for MVTV channels. I agree to return the equipment at the time stated. I understand that I will be subject to late fees, membership suspension, an/or other disciplinary action as a result of a late equipment return.
